Drug use articles making news – parents beware.

Over the last week, a lot of articles have appeared in the news about the effects of new trends in drug use. In a previous post, I talked about new drugs and drug trends. Here are the latest developments on that front.

Drug use during pregnancy.

A recent study reported that there are more and longer-term effects of methamphetamine use on children. My experience in counseling in California’s central valley led me to expect to see more of this and now we are. A large multi-site study shows that children with perinatal exposure to Meth have long-term problems with behavior. The symptoms look a lot like ADHD but include lots of other behavior problems and learning disabilities.

Counselors have learned to be skeptical of these kinds of studies, but this one is different. A lot was made out of the phenomenon of “Crack babies.”  Long-term studies were not as certain. Crack babies were being born prematurely and with low birth weight, but given good care, they did catch up. Many of the early problems in “Crack babies” diminished as the child matured. Not so with meth babies.

Meth babies are often born prematurely with low birth weight also. Yes given good care, they put on the weight and mature also. But unlike Crack babies, the Meth children do not seem to be outgrowing their problems.

Now in all fairness and the interest of honesty here, pregnant women who knowingly use meth during pregnancy are not the largest part of the problem.

The injury done to the unborn by the mother’s use of drugs depends on the point in the pregnancy at which the mother consumes the drugs. A whole lot of damage gets done during the first trimester of pregnancy when women do not yet know they are pregnant. This is particularly an issue for young women who may underestimate the chance of getting pregnant for the first time.

We should also remember that no matter how serious the damage which was done to the unborn by meth or cocaine, there is another drug that is doing more damage. One drug alone accounts for more than half of all the avoidable birth defects – and that one drug is – Alcohol.

Kratom use is picking up in the U. S.

This drug is banned in its native country but is being produced in the U. S. now. It is also more available than ever via the internet. Like most other drugs that some people use to get high, this drug now has a fan club that wants to promote the benefits of “Medical Kratom.”

Synthetic Marijuana use goes up.

Doctors in emergency rooms are seeing and sometimes missing overdoses of synthetic marijuana. Clients in substance abuse programs have been caught smoking these products to get high because they think they will not get detected by drug tests. The synthetic marijuana’s does show up in the toxicology screens but E. R. doctors are missing this because the symptoms can vary so much from one person to another. Over ten percent of high school students are reporting they have smoked synthetic marijuana. Symptoms go anywhere from a mild high to psychosis and catatonia. Hearing voices and not being able to move is not what people expect from smoking this drug but it can happen.

Energy drinks send their users to the E. R.

Getting your energy from a bottle or a can may not be all we thought it was. Hospital emergency room visits for people, mainly men, under the influence of energy drinks has gone up 1,000% since 2005. Some brands contain not only caffeine but lots of other stimulant chemicals. When the stimulants kick in especially when combined with coffee or other caffeinated beverages the symptoms can look like a heart attack.

Even more serious is the combination of energy drinks and alcohol. People think they are ok because of the stimulant in the energy drink but are impaired by the alcohol without knowing it.

Khat may be banned in the Netherlands.

Now when a country with notoriously liberal drug laws bans a new drug we have to wonder what is up with this one. Some people are suggesting that the reason the Netherlands is banning Khat is racially motivated, most Khat users are also ethnic Somali or from eastern African. The official government version is that this is not about race but because of the problems they are seeing with those who use Khat on a regular basis.

The liberal drug laws of Amsterdam may be fading away. What the Netherlands has found is that while liberal drug laws may have helped control some diseases in the population it has also attracted a lot of drug addicts from other European countries. Some articles suggest that even Amsterdam is no longer enthralled with being the drug tourist attraction it once was.

So there you have it. Some updates to my last post about new drug trends parents should keep in mind.

Love triangles and threesomes

Are you in a love triangle?

Those old love triangles are the stuff novels are made of and I expected to see a lot of them in the work I do as a Marriage and Family Therapist. Threesomes and triangles conjure up different images but the issues that turn up in the counseling session are very similar. One particular variety of this theme is especially common in my counseling practice.

The classic love triangle has love flowing around the outline but not back in the direction it came from. Usually, this is two people in a relationship when one decides they would prefer to be in another relationship. If they are married, or they have children this gets messy. The worst part of these affairs is when one partner wants out and the other wants to save the relationship. Often there are a lot of hurt feelings and when these relationships end there can be violence, depression, or self-harm. This is common but it is not the relationship with three parties I see most often.

The relationship that jumps to mind when we say threesome is more like three people involved in sex or a close love relationship presumably with the knowledge and consent of all the parties involved. I can’t tell you if this is a particularly common occurrence as I don’t do research on sexual practices but when it does lead someone to counseling, it is usually because it did not turn out to be all fun and games. When these relationships fall apart there are likely to be three or more hurt people. But this is far from the most common three-way relationship I see in counseling.

The most common threesome type relationship that drives clients to counseling involves two people, sex or sexual orientation doesn’t matter here.  What does matter is that one of the parties has introduced another love object into the relationship? That love object is far and away most likely to be some form of addiction.

Falling in love with a drug.

Sherry or Jack or Bud is likely to be a constant visitor in many a relationship. Men and woman will forsake their partners to be with Crystal or Crack. And members of both sexes are likely to fall head over heels for lady luck. Addictions of all sorts become a part of a couple’s love relationships on a very frequent basis.

Some partners try to compete with the addictions intrusion in the relationship. Clearly, over time the formerly loving partner becomes alienated by the new-found love interest. Who can compete with the wiles of Ethel-nol? As the struggle for attention increases, all sorts of family dysfunction become the norm.

Many a couple has thought that the way to tame an alluring addiction is to invite them into the relationship. Turn your partner’s new love addiction into a family affair and make it a threesome. This works for a while. Life is a party that seems to never end. An addiction, whether Crystal or lady luck, is a jealous mistress and eventually she takes over and turns the whole family into her servants.

Addiction stays to console the addict long after the family, loved ones; job, and health have abandoned the house. Many an addict has given up their house to stay with their addiction.

By the time the unwary couple calls the counselor seeking treatment both partners, the relationship, jobs, and children will have been destroyed.

When addiction has become a part of the love triangle the only cure is a full divorce from the addiction before all is destroyed. Unfortunately, by the time the addict, alcoholic, or gambler leaves their best friend, their addiction, the damage may have reached the irreversible point.

The moral of our tale is that when addiction is a part of a relationship there is not much left for anyone else. The earlier you seek treatment because you are in love with an addiction the better. If someone you love has fallen head over heels for an addiction insist they end this affair before everyone suffers.

Seek help early if addiction is a participant in your relationship.

Stoned Driving on Marijuana.

You are more likely to be involved in a fatal car crash if you drive while under the influence of marijuana. Being stoned behind the wheel increases the risk of an accident even when there is no alcohol present in the driver’s system. We found this out not with one small study but with nine studies that together included over 50,000 people.

Driving under the influence of marijuana about doubled the risk that you will be involved in a motor vehicle crash. This analysis didn’t come from the DEA or a “law and order group” but is from a study recently published in the British Medical Journal.

This isn’t the first time studies have shown this result. In a 2009 report from the National Highway Traffic Safety Administration, they reported that almost ten percent of drivers were under the influence of marijuana. They also concluded that over sixteen percent of drivers had drugs of all kinds in their system. Till now it has been easy to doubt that it was the Marijuana that was causing the impairment because older studies did not separate out people who were impaired by other and by multiple drugs. The British study solved that problem by looking at the risks of people under the influence of marijuana only, as opposed to those who were not impaired by any drug at all.

Why did they find that marijuana impaired a person’s ability to drive? We know that alcohol impairment is a serious problem. Alcohol interferes with driver’s awareness of speed and it reduces their reaction time. I tell students that a simple way of understanding alcohol Impairment is that alcohol shuts down the part of the brain that tells us “Hey stupid don’t do that.” What does marijuana do?

Marijuana alters people’s perceptions of space and time. Marijuana affects “special location.”  Stoned drivers follow too close, misjudge how close they are to the car in the other lane, and tend to swerve in and out of traffic often cutting the distance too close. Additionally, drunk drivers tend to know they are impaired and they often slow down. Sometimes they slow down way too much. Stoned drivers don’t do that. They can’t tell how fast they are going or how far away they are and they don’t feel that impaired.

The problem of stoned drivers is not a small thing. We have begun to recognize just how often serious automobile accidents are the result of impaired drivers. Just because a drug is prescribed does not make it safe. People are getting arrested for driving under the influence even when they have prescriptions for the drug. The fact that some people have prescriptions for Medical Marijuana, has lulled many into a false sense that this drug has no downside and is safe.

I continue to have conflicts about the use of Medical Marijuana. There are those who swear by it and report great medicinal befits. I am all for reducing human suffering and increasing happiness. But the idea that your medicine might kill me or someone I love makes me skeptical. I think people on both sides of the Medical Marijuana debate have left out things that we should know. I plan to write more about this in the future. But for now, I would conclude that if you need to use Medical Marijuana please don’t drive.

Third Hand Smoke.

Have you ever heard of third-hand smoke?

Most people haven’t. There is a lot more awareness of second-hand smoke these days, but not third hand.

It took us a long time to recognize the health damages of smoking. There are still some people who try to argue that smoking is not harmful to health. They can point to some old person who lived a long life and smoked every day. They can also show us instances of people who never smoked and developed lung or another cancer anyway. But the studies continue to pile up that show that for any group of people we look at, the non-smokers, overall, have fewer health challenges than the smokers.

Smoking mental health connection.

This is especially important to me as a counselor because there is a strong connection between smoking and mental and emotional disorders. Substance abusers and the mentally ill smoke a lot, as any counselor who works around this group can tell you. It is not just the act of smoking that causes problems, which is where second-hand and third-hand smoking comes into play.

Now cigarettes are not the only problem. Areas with high air pollution and poor air quality are likely to have increased rates of people with respiratory problems. Anything on fire is likely to cause problems. One stud, I read, said that people who burn a lot of incense can develop respiratory problems also. Sure smoke from a poorly set fireplace can fill a room with smoke, though most of this goes up the chimney. Forest fires and structure fires are not good for the lungs either. But cigarettes are the major cause of indoor air pollution.

Smoke goes where it wants to.

The smoke, fine particles, and hot gas from the cigarette, are not the only problem. The basic chemicals in cigarettes are also significant issues. Tar and nicotine in the air don’t know to only go into the smoker’s lungs. They float around and fill everyone in the room’s lungs. Now we know about the risk of second-hand smoke in restaurants and bars. We are also connecting the dots that show that kids who live with smokers have more asthma and other breathing problems. Parents and other adults should not smoke in an enclosed space where kids, the elderly, or other susceptible people are present. Waiting till the kids leave is not enough though.

Where is the smoke when you can’t see it?

Eventually, the “smoke” in the air disappears. Where does to go? Some of it sticks to the walls and the ceiling, a lot of it settles to the floor. Now that residue, that tar, nicotine, ash, and other nasty stuff waits patiently in the carpet and stuck to the floor.

Eventually, someone walks on the carpet, maybe a child crawls around in the shag. This could be weeks or months after the smoker has left the room.  The longer the smoker was in the room the more gunk is in the carpet. Shampooing and vacuuming get some of it out but there is still plenty left.

When that child crawls around they are stirring up a lot of that residue. They are only inches from the surface and the little particles find their way into their lungs. Now they are exposed to the smoke of a smoker who has not lived in the house since they moved in. They may never have met that smoker but they are impacted by that residual smoke.

So there you have it. Third-hand smoke is breathing in the residue from smoking that is stirred up from surfaces long after the smoker has left. It may not be possible to ever completely decontaminate the room. This has led to a movement to keep some rooms forever uncontaminated by anyone who has smoked.

We would like to think that this would solve the problem. But we know that despite all the warnings parents still smoke in the car with their kids present and we know that if they smoke in the house even when the kids are not home those kids and other people long after will be exposed to the toxins from that smoke.

Quitting cigarettes is hard.

Now I know that quitting smoking is not easy. Former Heroin addicts tell me it was easier to kick heroin than cigarettes and I believe them. But maybe the idea that long after the smoker has gone their children or grandchildren may still be breathing in the results of their smoking will spur some people to try to quit.

This blog is about having a happy life. So if you chose to keep smoking that is on you. But as hard as it is to quit I can’t think of anyone who has stopped smoking and stayed quit that has ever told me they regretted quitting. That is my encouragement on making the change if you decide to do so.

Do drugs cause mental illness?

What is the connection between drugs and mental illness?

By drugs here I am talking about illegal or street drugs, though prescription drugs can be misused and create the same effects. The relationship between drug use and mental illness is not so clear-cut as first glance would suggest. Not all mentally ill people use street drugs and many of them did not try street drugs until after they had an episode of mental illness.

1. Drugs can cause a mental or emotional problem.

There is a clear connection between some drugs of abuse and some mental emotional and behavioral disorders. We call these problems “substance-induced disorders.”

That connection can include emotional problems caused by prescription medications. Stimulant abuse, especially Methamphetamine can cause full-blown psychosis. Currently, we think there are several connections between Marijuana and psychosis. That would be a subject for another post, maybe even a book.

Drugs of many kinds can cause or exacerbate depression and other mood disorders. They can also cause or increase anxiety. There are several ways drugs can cause an emotional problem.

A. Intoxication.  People may behave differently when under the influence. Alcohol is an easy to see case of this. So is Phencyclidine abuse. People who rarely have caffeine may have an intoxication reaction if they suddenly ingest a large amount.

B. Withdrawal – They may have problems when they are withdrawing from drugs. Heroin or opiate-dependent people get very physically ill. Meth users crash, become depressed, and sleep for extended periods of time.

C. Substance-induced disorders. They may also develop long-term issues as a result of drug use. Meth-induced psychosis is becoming more common in my area. The central valley of California was described in a recent news article as “The Meth Capital of America.”  It is clear that people under the influence of meth can be psychotic. It is also beginning to be clear that the psychosis does not always go away once they stop using Meth. The way in which Meth damages some parts of the brain more than others, those changes on brain scans, are making some researchers look at specific structures in the brain as possible causes for many cases of psychosis.

There is a difference in the hallucinations experienced by drug abusers and those reported by people with psychosis. People with Schizophrenia most often report hearing voices, auditory hallucinations. Meth-induced psychosis is most often visual. They see Meth monsters or pet shadow puppies. Meth users also report that their visions do not go away when they close their eyes. So while the two types of psychosis are similar they are not identical. Maybe future research will answer all these questions.

2. People with a mental or emotional problem may be more attracted to a particular drug.

People with chronic episodes of depression may be attracted to stimulants. The quick lift of caffeine or cocaine can make your depression more manageable – temporarily, very temporarily. Freud, the father of modern psychiatry experimented with Cocaine as a way to treat depression and produce overall happiness. What we found out was that while there might be a lift of mood, there was always a crash afterward.  So the use of stimulants is no longer accepted practice because it leaves the user in a worse state than where they started.

Other examples of ways in which people with particular mental illness might be attracted to a particular drug are the way in which people with psychosis, meaning schizophrenia, schizoaffective disorder, sometimes Bipolar disorder, etc. are highly attracted to smoking. The nicotine may have some effects on the disorder. I am not suggesting anyone should take up smoking; the bad health results far outweigh the current pleasant feelings.

While we are on the subject of smoking, however briefly, I need to mention one report I read that said that more than half the cigarettes smoked in America are consumed by people with a DSM-4-TR diagnosis. Some writers have suggested that cigarette companies have deliberately added things and marketed their products to people with mental illness and substance abuse disorders. I do not know if that is true but I do know many clients with severe mental illnesses and substance abusers are heavy smokers. Remember this blog is called counselors soapbox for a reason.

In residential substance abuse facilities, we find that many clients entering treatment had the symptoms of a mental or emotional disorder before they ever started abusing substances. One theory to explain this is that some substance abusers are “self-medicating.”

3. Giving up your drug of choice can result in severe depression, anxiety, and other emotional problems.

Most long-term users get closer to their drug of choice than to other people. All their friends are involved with the same drug. So once they quit they are very alone.

This process of giving up your drug of choice is a lot like grieving over the loss of a family member or friend. The alcoholic might tell you that women come and go but “Sherry is always there.” The Meth user describes their effort to quit as “Crystal is always there.” And they remind us that “She,” whichever she that is, is jealous and didn’t want them having any other relationships.

So there you have some thoughts on the relationships between drugs and mental illness. There are other relationships but I need to stop for now.

7 “New Drugs” parents should be aware of

7 “New Drugs” parents should be aware of.

New drugs and new patterns of drug use continue to emerge. When I first started talking about this in my class for drug and alcohol counselors the thought entered my mind that giving out information on new drugs might encourage their use. I didn’t need to worry about that, the people who want to use them knew about them far before I did. But now that there have been a few overdoses I believe it is important for parents and professions to be aware of these new trends in drug use. So here are six new drugs and one new drug use trend that are beginning to rival the old drug use problems. For up to the minute information on these drug use trends you only have to search the internet.

1. Khat

Khat is a stimulant plant from the Middle East, another of the results of our involvement in wars there. The leaves are chewed while still fresh and moist and are a mild stimulant similar to Coca leaves. The plant and the fresh leaves are rare in the U. S. The synthetic version is becoming more common.

2. Methcathinone

Methcathinone is a synthetic and potent laboratory-produced version of the Khat plant’s active ingredient. It is not illegal or regulated everywhere yet and is growing in popularity. Results are reported as being similar to Methamphetamine.

3. Bath salts.

These are not the kind of bath salts your grandmother might have used. These drugs are being sold in “head” or “smoke” shops not bath boutiques and the label is certainly a misnomer. Common names include such benign-sounding names as “Ivory Wave,” “Purple Wave,” “Vanilla Sky” and “Bliss.”

They are used by smoking and can contain a variety of chemicals. Join Together reports that DEA has placed a temporary ban on three ingredients used in the manufacture of bath salts, Mephedrone, MDPV, and Methylone.

As fast as one ingredient is made illegal the manufacturers switch ingredients. Overdoses can be particularly nasty and use may result in psychoses or death. To re-quote “What a price to get your kicks.”

4. Synthetic Cannabinoids.

These are best known locally under the brand names of “Spice” and “K-2.”  This can be most any dried vegetable material, commonly parsley which has been coated with a synthetic Cannabinoid. There are 300 different chemicals involved so far and more are sure to be discovered. A few have been made controlled substances, mostly this means they are illegal in the U.S. As fast as one is banned another variety comes into use. These are not benign chemicals. Overdoses and toxic results have been reported including hallucinations that have not gone away after withdrawal from the drug.

5. Salvia Divinorum.

An unusual member of the sage family originally from Central America it appears to be the only member of the sage family with psychoactive properties. It was used by Native Americans in religious ceremonies and does not appear to be especially dangerous when used that way. When combined with other drugs, especially synthetics and alcohol the results are reported as being unpredictable. Since stimulants and depressants are the most popular drugs, consciousness-altering drugs like sage have not caught on in popularity the way Methcathinone and bath salts have. As with most dried herbal products the potency and ingredients can vary considerably.

6. Kratom.

Bet you thought I made that up? This has nothing to do with Superman. It is a tree, originally native to South East Asia. The leaf is reported to have both stimulant and depressive properties. In some places it is illegal and in other places, it is totally unregulated. At high doses, it has been reported to have effects similar to morphine. Some of the trees are now in the U. S. but most of the use is by buying leaves and preparations from the internet. The tree does not grow well in cold climates so most of the cases reported are from Florida. Like all other drugs, it is likely to spread over time.

7. Smoking of Heroin by teens.

This is a new twist to an old drug. This trend is occurring in the wealthier and more affluent parts of town. Abuse of pills is now epidemic. Teens have ready access to powerful painkillers. Sometimes these have been prescribed to them for injuries but often these pills are being taken from parents and grandparents medicine cabinets. After a short time, opiate addiction develops. Unable to get more pain pills an exceptionally large number of teens have taken to purchasing heroin to replace the pills. At first, they may be induced to smoke the heroin, thinking that this differentiates them from the drug addicts who use needles. The high price of the drug and the larger quantities needed when smoking result in most switching to needle use. Heroin is consuming a whole new generation.

By the time I get this posted it is likely there will be additions to the list. I hope this helps in the way of information. The only antidote I know of for an increase in drug addiction among our children is parental and societal involvement with kids. Happy, healthy kids are less likely to become addicted and they are more likely to turn to adults for help. Kids with mental health problems, who are estranged from their parents, are at increased risk. Trying to keep drugs out of our communities does not seem to be working as you can see here new drugs of abuse will keep entering our society. The only hope for taming the dragon of addiction is early intervention and treatment.
