Is stress a diagnosable reason for time off from work?

Stress and time off from work – Morning Question #18

Short answer – Yes, no, and maybe.

Yes. – Stress is diagnosable – IF the person’s reaction to the stress is extreme and they are unable to work, have relationship problems, or are upset about the way the stress is affecting them. It could be Acute Stress Disorder, Posttraumatic Stress Disorder, or possible an Adjustment Disorder.

No – not everyone who has stress at work should get a diagnosis. Everyone has some stress, even good things like a new job or a promotion can be stressful. So just having stress is not enough for the diagnosis.

Maybe – Maybe you can take stress time off with pay, maybe you won’t get paid for time off.

Time off from work is a legal issue. If you have leave coming or qualify for disability then you might get paid time off. Sometimes a counselor will suggest it would be good for you to take some time off even if you do not qualify for paid time off. Self-employed people probably will not get any paid time off but the counselor may recommend it anyway.

Check the laws in your jurisdiction and the rules for your workplace regarding the issue of the time off being paid.

Does an adjustment disorder produce depression and mania?

Morning Question #11 adjustment disorder, depression, and mania.

Adjustment Disorder is essentially an excessive reaction to a normal stressor. The DSM-4 currently lists 6 types of adjustment disorders. They all involve some combination of anxiety, depressed mood, or behavioral problems.

Once the reaction to the stressor becomes enough to cause symptoms of a Major Depressive Disorder that is what gets diagnosed. If there is any mania or hypomania, that would become a Bipolar Disorder.

Stress can trigger all sorts of mental health issues. Someone with a history of depression or mania or an underlying risk factor like family history could have an episode under stress.

So rather than adjustment disorder producing depression & mania, stress could cause any one of the three.

Does sleep paralysis cause or is it caused by mental illness?

Remember this is written from a therapist’s point of view. If there is any chance that you or someone you know has a medical problem, have it checked out by a doctor first. But if the doctor finds nothing medically wrong with you that does not mean you are going crazy. It might mean that you can benefit from some counseling to help you better cope with stress.

Many people who experience sleep paralysis, hypnogogic hallucinations, or “exploding head syndrome,” think they are going crazy or fatally ill. Knowing that these are explainable phenomena and have simple treatment can reduce the concerns. Sleep Paralysis and many related sleep problems are often triggered by stress. More stress, good or bad stress, and the chances of an attack increases.

People who become fearful of another occurrence of Sleep Paralysis can “prime the pump” and increase the risks of a second bout in the same way people who experience panic attacks begin to worry about having another episode.

If clients describe these events as dreams the doctor is likely to reassure them that it is normal. Patients who explain these events as demons, spirits, or believe they actually saw a supernatural being are likely to be prescribed a psychiatric medication. Antipsychotics, antidepressants, and anti-anxiety medications (Benzodiazepines) are all believed to increase the incidence of Sleep Paralysis and Hypnogogic Hallucinations (Gangdev, 2004.)

Other things that have been reported to increase the risks of having an episode of Sleep Paralysis include being physically ill, such as having the flu, watching or experiencing emotionally upsetting events, such as having an argument.

If the paralysis or hallucinations only occur when going to sleep and waking up they are most likely sleep-related and not the result of a mental illness. Gangdev, in his article, asked the question: “It is possible that a small proportion of patients diagnosed with schizophrenia who experience hallucinations may actually be experiencing escaped REM-related dream activity during the wakeful state?”

There is a significant overlap between sleep paralysis and Narcolepsy. Narcolepsy includes not only sleep paralysis but hypnogogic and hypnopompic hallucinations, daytime sleepiness, and Cataplexy (sudden unexplained loss of muscle tone.)

Sleep Paralysis without any cataplexy or daytime sleepiness is not considered to be associated with Narcolepsy and is referred to as Isolated Sleep Paralysis (ISP.) Penn reported that 16 % of medical students reported at least one episode of sleep paralysis. That makes me think that long hours and sleep deprivation may be a major cause of many of these events.

Sleep Paralysis is far more common in African-Americans and in one study of Nigerian subjects more than half had experienced ISP. It is also common in Japanese Subjects.

People who have a Sleep Paralysis event find it helpful to get up move about and make sure they are fully awake before attempting to return to bed. People who do not get out of bed have an increased risk of having multiple episodes of sleep paralysis in the same night. Sleeping flat on your back looking up at the ceiling (supine position) is much more likely to cause a Sleep Paralysis experience than sleeping on your side.

Knowing that episodes of Sleep Paralysis and Hypnogogic Hallucinations are relatively common and most often harmless can help someone cope with these experiences.

Acute Stress Disorder vs. PTSD

What is the difference between stress, Acute Stress Disorder, and PTSD?

Stress is a normal human reaction to something that threatens us or challenges our ability to cope. When challenged our bodies to respond. Even good things we have looked forward to, like new jobs, marriages, or the birth of a child, can cause stress. Negative events, loss of a job, divorce, sickness, or the death of someone we love can be even more stressful. Stress is a normal part of life unless it gets out of control.

Acute stress disorder is when something stresses us out and this stress results in impairment of our ability to function. It goes far beyond just being stressed out and needing time to recuperate.

Acute Stress Disorder is a diagnosable mental illness, though much of it goes untreated and unnoticed and like adjustment problems or a mild depressive episode may go away by its self, untreated. If it becomes severe enough it needs treatment before the symptoms get out of hand. The symptoms of Acute Stress Disorder must last for at least two days and must happen during the thirty days after the stressful event. Once the episode lasts more than 30 days we reclassify it as PTSD. PTSD may also intensify and produce symptoms that are in excess of those seen in Acute Stress Disorder.

Acute Stress Disorder.

Acute Stress Disorder is not just being stressed out or needing time off from work, it is far more debilitating than those symptoms.

Here are the things that need to happen for the stress to be Acute Stress Disorder. This is in my words, not the official DSM language.

Warning: This description is not meant as a diagnose-your-self project. If you think you recognize yourself, a family member, or friend in these descriptions you really should see a professional.

1. You experience or see something that makes you afraid you or someone close to you like a friend or family member will be killed or seriously injured. This could be an actual event or someone who threatened you and you believed them. As a result of this harm or risk of harm, you become intensely fearful, helpless, or horror-struck. Note this is pretty bad stuff, not just being chewed out by your boss or the risk of being fired. Those milder things are stressful and might result in an adjustment disorder if they affect you enough, but those non-life-threatening things don’t get called Acute Stress Disorder or PTSD.

2. You get lots of DISSOCIATIVE symptoms – 3 or more. Dissociative symptoms, those are bad.  Here is the list:

A. Numbing or not feeling emotions

B. You don’t feel like things are real – called derealization

C. You can’t be sure you are you – called depersonalization

D. You get dissociative amnesia – can’t remember big chunks of what happened.

3. You keep reliving this experience, like that episode of Star Trek where every day they got up and it all happened again. Your life turns into a rerun.

4. You would go a mile out of your way to avoid places or people like that again.

5. You are on edge, jumpy and the least little thing sets you off again.

6.  You get so upset you can’t go to work, avoid friends and are afraid to talk about this let alone ask for help.

7. This experience and all its terror lasts 2 days to 30 days.

8. By the way, if you did bad drugs and imagined this or there is something medically wrong with you – forget all the above and get to a doctor right away.

So what makes it PTSD?

If you have the above and it goes on over thirty days we change the name to PTSD. But then the longer this goes on the more the symptoms. This is one reason we are thinking that if we could get to people who have been injured and treat them right away we just might keep this Acute Stress Disorder from turning into PTSD. That means treating some people who could get better on their own without treatment in order to prevent others from getting PTSD, but given the long-term debilitating results of PTSD, a little extra treatment might be worth it.

Not sure what you think, but I believe that if we could provide appropriate services to all those returning GI’s from the Middle East we could prevent a lot of long-term suffering.

Those guys are worth the effort right? For the accountants out there, prevention saves a lot of money on long-term treatment also.

As Acute Stress Disorder goes past the 30-day mark lots of more severe symptoms develop, nightmares, extreme efforts to avoid anything that might remind them of the trauma, and lots of drug and alcohol abuse. PTSD and alcoholism are best friends.

There is disagreement right now about the best way to treat PTSD. I will write more in future posts on PTSD, stress, and some available treatments and new approaches that sound really interesting.

Meditation for people who don’t meditate

Are you a meditator or a non-meditator?

For a very long time now I have been convinced that there are two kinds of people, meditators, and non-meditators. I was sure I was a non-meditator. Turns out I just might be a closet meditator after all.

There are lots of meditators out there. I read about them and the benefits of meditation. I am sure it must be immensely helpful to those who practice it, but like weight loss and exercise it has always on my “to do” list not my “done” list till now.

Meditation fits nicely with all those pleasant eastern religions, but I am a hopelessly western person. How could meditation fit into my life? I have been to a twelve-step meeting where they talk about prayer and meditation but getting into meditation has been an awfully long stretch for me.

One problem with my meditation efforts may have been the tendency to think that somehow I needed to close my eyes and block out the world. That is the way we are supposed to pray in church so that was the way I tried to meditated. The same problem occurs with both prayer and meditation when I close my eyes. First, my mind floods with thoughts about everything in the world. These are important thoughts, creative thoughts and I don’t want to lose them so I keep trying to remember these important ideas while trying to empty my mind. Ever try to empty a large pool of water using a running hose?

This flood of ideas happens at other times, like when I am supposed to be writing my blog post. At those times I use a “capture tool” for those pesky other thoughts. Rather than trying to remember the idea I just got for tomorrow’s post while typing today’s installment, I write it down on a clipboard next to my computer. This “captures” the thought and lets me drop it out of my mind and concentrate on the idea at hand. I tried that with meditation but it seems disrespectful to the meditation leaders to always be writing things down while others are trying to meditate. Seems downright sacrilegious, in a non-religious meditation way.

My training as a counselor has been mostly centered around the western style of cognitive behavioral therapy. Get a head change, change your thinking and the emotions will follow. Meditation might work for some of my really anxious clients but I was a little unsure how to teach them the benefit of something I had always been sure I didn’t do – till now.

Someone recently told me they meditate by watching the leaves in the trees. There was a brief flash of thought, not willing to call it enlightenment just yet, but I started to wonder – could watching the leaves in a tree be a form of meditation?

In counseling, we work on a skill called “attunement” with clients. We try to not only get the meaning of their words but also the feeling behind those words. We try to see the world from the client’s point of view. If we can attune to people why not trees?

It suddenly came to me that the times in my life which were the most peaceful were when I could attune with something in nature. Sitting on the ground staring away at the leaves, watching the wind making its way through the tree and feeling attuned to the tree, that occasion was one of the most peaceful times in my life.

Trees have a lot to tell us. From a distance, they look a lot alike. Close up it is amazing the variation. No tree is perfect but you don’t need to be perfect if you are a tree, you just grow and give shade.

Like most people I have moved all around looking for the place I belong, that tree outside my office has lived in that same spot since the day it was born. In all likelihood that tree will still be here after all the people who work in the building have come and gone.

The tree has its struggles. Last winter there was a wind storm and the tree lost a branch, broke off all of a sudden. The tree lost part of its self and still just kept on growing, reaching for the sun. We humans sometimes stop growing after a loss like that.

Trees aren’t the only non-humans we need to attune with. There are birds that act out their drama while living in that tree. The rain comes and it plays with that tree until drops fall to the ground and flow away.

Watching the river run downstream is also a peaceful centering experience. Water, by the way, does not care if you watch or close your eyes to meditate. If you close your eyes while attuned to a river, it will sing to you.

There you have it. While I will probably never be able to sit quietly staring off into space and meditating, I find great joy and contentment in watching the wind play games with the tree outside my window and the rain run down the stream to the river and eventually the sea.

Could you accept that attunement to nature is a very productive form of meditation?  It’s a version of meditation that works for me.

Thanks – I think

Even when I don’t agree with someone I appreciate their interest in my blog. Links are even better. I think it was La Guardia that said, it does not matter what you say about me, just spell the name right, or in this case get the link right. But still – something that happened today shows how easy it is to be misleading when you quote someone else. In this case, they were sort of quoting me.

My blog about our tendency to take the easy way out and give kids lots of meds instead of the personal connections they need to develop was quoted on a website about Medicare. That was nice, only the picture they ran with my blog post did not come from my blog which might make it misleading.

Just so everything is clear, I am very much in favor of parity laws. Paying as much to treat mental illness and drug addiction as we do for physical illnesses is an idea whose time has come. I am also in favor of anything that makes sure health care coverage is available to all.

People who do not have health care end up at free county hospitals and most of the time they don’t get to the emergency room until they are critical. People with untreated mental illnesses are clogging up our jails and prisons and medical hospitals. We pay for all this treatment whether the people have insurance or not. So some form of national healthcare preregistration just makes the system more efficient and manageable.

It seems clear that the country cannot retreat to the days of family farms and paying the doctor with chickens. Our future also does not involve making more things for cheaper prices. Our future as a country does depend on developing minds. We need more Ph.D.’s not more press wood furniture.

Some people seem to think that teachers, especially college ones are a waste of resources. They don’t want to pay for medical services for anyone but themselves. If this country is to stay a world leader we need more teachers, more doctors, and more professionals who work in mental health and substance abuse. We also need more engineers and scientists. Developing these specialties requires easily accessible education as well as physical and mental health care.

All of these things, like the military and the coining of money, are things that need to be undertaken by the government for the good of all. The majority of promising candidates for the service industry do not come from the rich. The best and brightest minds come from among the ninety-nine percent and they need the government at all levels to promote the growth of the service industries and education.

The future is in the areas of ideas and services. The government needs to take the lead in growing these sectors. I am not opposed to businesses having a large role in this effort. Non-profit companies are especially good at efficiently providing these services. But the mentally ill, the drug addicts, and the homeless don’t have money to pay for their care and we all pay for it one way or another.

So the association of a picture of President Obama superimposed with a communist flag was not my sentiment. If anything, I think this administration has been too conservative in accommodating the role of large businesses in the economy.

We made progress in the space race because the administration at that time led the country into a new area. Hopefully, our leaders will lead again into the new service-oriented economies of the next few centuries.

The one bright spot in all this, to my way of thinking, is that at least a couple of Republican candidates for our highest office are now sounding like they are more liberal than the current administration. Maybe this veer in the liberal direction will get our nation back on track.

The hummingbird picture came from Wikimedia. It has nothing to do with this post but then the picture someone attached to my last post quoted on their website had nothing to do with my post. So now things are about even.

Stress!

“Can you feel the stress in this room?”

The person next to me bent over and whispered in my ear. “Yes,” I replied. “I have never felt it this strongly,” they said.

Are you feeling it too?

These days people are collapsing under stress as never before. Fear is stalking us.

We are daily reminded of two wars – should we make that three – and counting? Housing prices continue to crumble; most homeowners are underwater if they haven’t lost their home to foreclosure already. Unemployment is up – stubbornly up. In my area, unemployment flirts with the 20% mark. Is it any wonder we are stressed?

The gridlock in Washington hasn’t reduced the stress level of anyone I know. Did I hear that right – politicians almost let us default on the NATIONAL DEBT! And the stock market makes my stomach hurt worse than any roller coaster I ever rode on. Consumer confidence is near the bottom. Did I read that right – one credit card company emailed me a warning – I did not owe them anything but they were warning me THEY didn’t have the money to lend me?

Are company presidents really bragging about the number of American jobs they have eliminated?  It is not just the low-skilled unemployed that are out of work. People with college degrees are unable to find jobs; some are running out of unemployment benefits. And the once safe place – government employment – has become a scary place as huge numbers of government employees at all levels are being laid off.

There was a time when the way to escape economic uncertainty was to go back to school, usually to college, and get a degree. A degree is still a wonderful thing, but occupations are changing so rapidly a degree is no guarantee of finding a job.

California has traditionally been the land of second chances. And third and fourth … But as the budgets shrink colleges everywhere are raising tuition, cutting classes, and capping enrollment  So even if someone chooses to go back to school there are no guarantees of admission, classes, or a career afterward. That is causing students a lot of stress.

Being a parent is even more stressful. Lots of kids are not finishing school. The numbers going to juvenile halls show no sign of declining. Teen pregnancies and drug use are everywhere you look. If parents had known the risks beforehand we might not have become parents.

It’s only adults that stress right? Kids don’t understand stress, right? They don’t know we are stressed, we are hiding it well. Wrong! The hospital emergency rooms are seeing those kids. I read on the internet, a study which said over the last ten years the number of children admitted to psychiatric hospitals has continued to rise. These are kids who say they want to commit suicide, kids who wish they were never born, or they wish that they were dead.

No, not every kid says that, but the numbers who do are rising. Kids who never before knew what homelessness meant are learning the hard way. Kids tell me they have moved 3 – 4 – 7 times or more because the family couldn’t pay the rent. Several families move in together in a house too small for one family.

Some kids get kicked out of school. I am seeing a wave of home-schooled; self-taught students who couldn’t be kept in the classroom anymore because of their behavior – or their parent’s behavior.

Lots of those kids who end up in contact with the official system, welfare, Child protective services, juvenile hall, where are their parents? They are in prison and jail. In our society, for a long time, one in three black men went to prison. But the numbers are changing now. More Hispanics, more women, and more white men are going to prison. Nowadays everyone gets a chance to do time.

Our prisons are overcrowded and soon we will have to start letting people out to make room for the new prisoners.

Some kids are looking forward to seeing their parents freshly released from prison; some are scared for the same reason.

It’s the drugs, right? We have been fighting the war on drugs so long, our battle flag is torn and covered in blood. More children and younger ones are trying drugs. Some are doing drugs with their parents; others are stealing their parent’s drugs.

And it’s not just the same old drugs, six new drugs, maybe more, have arrived on the streets of America in the last couple of years. Some of these drugs aren’t illegal yet. Most don’t show on the hospital screening tests. And the kids who tell me they are using drugs – they keep getting younger and younger.

And the drugs that hurt the most are not the rare illegal ones. They are not the ones that sneak in through the windows but the ones that we openly carry in the front door. Alcohol and Tobacco-related deaths account for over 95 percent of all drug-related deaths. Abused prescription drugs are in second place with all illegal drugs a distant third.

So what causes these children to succumb to all this stress?  Why do some parents cave in under the stress and others make it through? In the future, I will write more about my thoughts on withstanding stress, but now I would like to know –
