The Inside and Outside of Relapse triggers.

Relapse triggers, either internal or external, are those things that set off cravings in a recovering person. The failure to do maintenance steps in the process of change increase the risk of giving into triggers.

External triggers are the things outside ourselves that place us at risk to resume old behaviors and give up on the progress of recovery. Shorthand ways of understanding these triggers are people, places, and things.

People are one of the biggest reasons people relapse. There is a huge temptation to look up old friends. Often the only thing that you have in common with an old friend is a history of using drugs or drinking. Sometimes there was a history of other dysfunctional activities, codependency, or abusive relationships. If the people you had around you in the past supported your addiction or if they were not affirming, or made you feel bad about yourself, being around them can take you back there. Avoiding people who are bad for you is not being selfish.it is being self-caring. In early recovery it is suggested you not make a change you can’t take back like changing jobs or relationships. Surround yourself with people who support your recovery.

Places are another important external trigger to pay attention to. Alcoholics need to avoid bars; drug addicts should avoid dope houses. But there are other places to avoid. People with relationship issues should avoid revisiting places they used to go with a partner who is no longer in your life. Should someone on a diet visit a candy store? I wouldn’t recommend it. Think about places that you may need to avoid if you want to be secure in your recovery. Is there a family member or former friend who triggers your issues?

Things can also reignite thoughts of returning to an old lifestyle. Music can be a powerful memory trigger, so can some smells. People with relationship issues, sometimes we call these people love-addicts, find it hard to let a relationship go. They keep the old moments out. They think about the things they did together. One last call to see how that person is doing is likely to set off a new round of problems. Carrying lots of cash can trigger some people, especially gambling addicts and former drug dealers. Sometimes it is a pipe or a lighter you find hard to get rid of. Is there something that reminds you of your issue but which you find hard to give up?

Internal triggers are the other part of the equation. The things going on inside our bodies and our minds are also relapse triggers. The word HALT standing for, Hungry, Angry, Lonely, and Tired is used as a reminder of those triggers.

Hunger, thirst, and many other physical sensations can make you feel restless, irritable, and unleash the cravings. Negative emotions are powerful relapse triggers. Feeling anger fear or resentment, any number of negative emotions can cause someone to catch a case of “who cares.” Loneliness sends people back to their disorder quickly. Being tired is likely to upset recovery also. All of these internal triggers have to do with not taking care of ourselves. It is a short hop from not taking care of yourself to thinking you don’t deserve care, after that why should you hang on? Why not go back to the old life? People who don’t provide good self-care don’t encourage others to care for them. They start believing they don’t deserve to be treated well and then they stop treating themselves well.

Another way of understanding internal and external triggers is to look at the two main causes of relapse, romances, and finances. Romances are all about your feelings, feelings of loving and being loved, self-worth, and self-esteem. Finances, mostly money, is the ultimate thing. Lack of money can sap our will to change. Having a lot of money makes some people feel they are invincible; the rules don’t apply to them. Pay attention to the healthiness of your relationships with things and with people. When one of these relationships gets out of balance, your life is headed out of balance.

These are only some of the things that might cause you to relapse. We each have our own triggers. What are yours? Knowing your triggers and how to defuse them strengthens your recovery.

Is Relapse a part of recovery?

At some point, every recovery program has to tackle the issue of relapse. Why after losing all that weight do people suddenly put it back on? Do all alcoholics; sooner or later pick up a drink? Once you have depression you will always have recurrent bouts right? The way in which relapses are understood separates the Stages of change model from almost every other model of recovery. I have been told more than once that relapse is a part of recovery. I am not buying that! It doesn’t have to be that way.

So if relapse were a part of recovery, every person signing up for a self-improvement program should get out their calendar on day one and plan for their relapse. Doesn’t AA even say a relapse is a part of recovery? No, it doesn’t. The AA book says that a certain percentage of people get it the first time and don’t relapse. Bill W. estimated that percentage at 50%. In the early days, it appears that a lot of people who really wanted the program and stuck with it got it the first time around. He also said that there were some people, maybe 25% who tried a few times and eventually got it. The rest were better while they were in the program but kept going in and out. The stages of change model agrees with that thinking.

So why do people say that relapse is a part of recovery? Mostly, people say that because relapse happens a lot. And if relapse does happen, rather than giving up, the objective is to pick yourself up and try again. One article I read said smokers typically have to try to quit seven times. Now some people quit on the first try, a few people become nonsmokers in a few tries and there may be some people who just don’t seem to be able to quit. Any self-change is like that. It takes more than trying hard.

It is not the quitting of bad habits that is the problem. Some people quit drinking 5 or 6 times in one day. It is the staying quit that is harder. So how does someone make a change, give up a bad habit, make an improvement in their life, and then stay changed?

The stages of change model says that after all that effort, that process of change, there is one more thing you need to do to stay changed. See change is not a one-time thing. Real lasting change is an ongoing process. Why do people think that they can excessive like crazy, take off a few pounds, and then once they lose the weight they can stop the exercise?  How many days off Heroin before it would be safe to try some drugs again? The truth is that change is not a thing – it is a process. Once you make the change you have to keep doing the process to maintain the change.

The thing you need to do to stay changed is called maintenance. To stay changed you will need to do some maintenance. If you mow a lawn once will it grow back? So if it took an increase in exercise or an improvement in healthy eating to lose weight it will require you to keep doing that work to keep the weight off. The addicted person can’t just put down the substance; they need to keep doing the things that got them sober.  In the stages of change, relapse is not seen as something that is part of change it is understood as a failure to do the one remaining step in the process of change – doing maintenance.

Now, this is easier to see when we talk about weight loss or addiction but what about depression or bipolar disorder? With any of life’s problems, there are things that you can do to make the problem grow and other things that make the problem shrink. Taking good care of yourself is especially important for mental and emotional issues. So if taking good care of yourself makes you less depressed, what might happen if you stopped eating and sleeping well? Your depression might grow. The maintenance step is needed for holding onto any change you made, it is the best antidote to a relapse, whether that relapse is weight gain, addiction, or depression.

Stages of change – Late Action

Late Action – The change accelerates.

Last time we talked about how I might be full of enthusiasm as my self-improvement program gets going. In the weight loss example, I begin to exercise and maybe even get interested in nutrition and diet. Are deep-fried Twinkies a healthy diet? The diet guru says no. I eat low-fat and lots of greens. I hate greens but I eat them anyway. My efforts at change are taking shape and encouraged by my own persistence I may even expand my efforts.

On days when I can’t get to the gym, workdays, I start walking on my lunch hour. It is a mile around the park. If I walk fast I can make two trips and eat during my lunch period. I discover that if I bring a healthy lunch, a sandwich, and some low-fat yogurt, and I walk around the park fast, some days I lose weight. Even if I don’t actually lose weight, at least those days I do not gain any more. At this point, the intensity of my workouts is increasing. I am really getting into this.

In substance abuse recovery terms, the person involved may not only be attending meetings, but they also have a regular home group they go to every week, maybe even a fellowship they attend every night. Some people “get into service” meaning they make coffee, take out the trash, and so on. We get trainers, sponsors, and a support system. At this point, my change is beginning to be something I tell people about.

For people who have an emotional problem, like depression or anxiety, their recovery action might be seeing their therapist on a regular basis, improving self-care, or journaling. There is a huge connection between emotional issues and eating problems. One of the key issues a professional looks for in making a mental health diagnosis is changes in eating and sleep. Binge eating, overeating or not being able to eat, as well as sleeping too much or too little are all symptoms of problems. They can also be causes. More about that mind-body connection and the relationship between sleep, eating, and other life problems in a future blog.

So in late action, I am getting somewhere on my self-change program. Losing a few pounds, not drinking or drugging and I am no longer so depressed I don’t want to get out of bed. Everything should be going fine. Right?

Then what happens? Why do so many people successfully make a change only to return to the place they were before? Why do most weight loss programs, diet, and exercise, end in putting on more pounds than we lost? Why do so many people get a thirty-day sober chip only to drink again? And how is it that depression and anxiety return after a period of time?

I start wondering, am I fixed? Do I have to give up those quadruple thick burgers with the pound of fries? How long will I need to take these psych meds? Can’t I just have a donut or a glass of wine? What kind of wine goes best with donuts? My mind starts looking for ways out of the change process.

For substance abuse, we call this relapse. For depression or anxiety, we are starting to think in those terms also. For weight loss programs the part that we don’t like to talk about is why after losing twenty pounds, do I put it all back on and then some. Every time I have been on a diet I have needed to get new larger clothes.

This is the point where people start talking to us about maintenance plans.  You mean I can’t just crash diet off ten pounds and then I will be able to eat like other people? Can’t you AA folks just teach me to control my drinking? So my depression is gone. I will never feel that way again. Time to get back to the way things were before my self-improvement program. So next time let’s talk about putting the weight back on, the relapse, and the return of emotional problems.

Stages of change – early action

Early Action – the beginning of visible change.

So here we are in that process of change at the point where we start to do some action steps. Up till now, we have had someone or something that created a reason to consider the need for change. No catalyst and probably most people would never embark on the journey. So we were pre-contemplative. Then we started thinking about the problem, gathering information. We called that contemplation. At some point, people get into preparation, join groups, buy books seek out resources. Still, no change has occurred, not real lasting change anyway. But now it is time for the do or die part, – getting into action.

Stage of change four – Early Action.

Some authors only have five stages of change and they have only one stage for action. I am separating action here into early and late stages as we find that the things people do at the beginning of their recovery process is often different from what they do later on. I got this formulation from Ken Minkhoff and Christine Cline at Zia Partners. You might want to check out their website and the Change Agent Movement they have spawned. The difference between early and late action is easier to see when people have multiple problems, like substance abuse and depression, this is a condition we call co-occurring disorders or complex clients. For someone with depression and a drug problem and a weight loss problem, in the beginning, all they may be able to do is get dressed and go somewhere. For them, this may be early action. Later they will be able to do more.

Now on with our story, first the weight loss example and then examples of other possible changes we might want to make.

So remember my story. I discovered others though I was gaining weight even though I hadn’t noticed. I weighed myself and gathering information and decided I needed to lose weight, maybe even change to some healthier habits. I joined a gym and bought some exercise videos maybe even some getting healthy books. Now I am at the point of starting to do something. I am ready to start exercising and eating healthier.

So at the gym, I seek out a trainer or a fellow member he shows me how to use the equipment, the routines, and the whole culture of healthy living. I like the hanging out and the talking about fitness, the exercises not so much but I do it anyway. Right now my goal is to lose that weight, get that old suit to button closed again, and generally shut up my friends who say I am fat.

At first, I wander around the gym. I try out a machine or two. I check out the weights to see what I can lift. I talk with people who show me how to do things. It amazed me how friendly some people at the gym where and how much they were willing to help a newcomer. The same things happen at AA or other self-help groups, people trying to help each other. In gyms and recovery centers there are professionals also, trainers or therapists, and counselors; they can be especially helpful.

So the workouts continue. The weight is coming off, a little anyway, and I am feeling better and better about myself. The scale still is not saying I have lost that much weight but I always suspected it of being out to get me. One thing I noticed was that even when I was not exercising at the gym, just wandering around, the whole time I was not eating. Even if I wasn’t losing weight I had stopped gaining.

If this were a substance abuse example the person would be going to AA meetings, getting a book, and a sponsor. They might start to share at meetings. There is an old adage that a closed mouth does not get feed so you need to talk as well as listening. Now just like the guy at the gym, the person at AA might still be having thoughts of drinking, but they start to notice that while they are at meetings, they are not drinking. If they can make it from meeting to meeting without drinking the periods of sobriety start getting longer.

Each different recovery group, AA, overeaters anonymous, gamblers anonymous, or weight watchers has its own culture. You need to find one that is comfortable for you. There are over 200 twelve-step groups now and hundreds more of peer and self-help groups. Some gyms program is about exercise and flexibility other groups are all about weight lifting and setting records. A group for cancer patients is not much help to an alcoholic unless they have cancer also. Find the group that is right for you.

Eventually I settle into a routine. I am doing the things I need to do to recover, but I am not there yet. I also am not sure I want to keep this up after I lose the weight. Here is where we start to hear the word maintenance and we also start to talk about relapse. The question becomes – is what I am currently doing enough or is there more. Many people quit drinking or take off the planned weight and then they are good with that. Others want a new healthy lifestyle. So what will it be for you?

So in my weight loss example, I have started an exercise routine, lost a few pounds, though the buttons on that suit are still tight. For a substance abuser, they have made it two or three days without their drug, maybe even three weeks. So now what? What will I do next? Next blog we can talk about how my weight loss or substance abuse recovery program may change as my action moves from a few times to a regular routine.

Recovery defined

What exactly is recovery?

Should we describe someone as “In recovery,” “recovering” or recovered? What about resilience. Is it in any way related to recovery? This month SAMHSA (Substance Abuse and Mental Health Services Administration) suggested a working definition for recovery.

The word recovery is commonly used when talking about substance abuse. Recently mental health has tried to incorporate wellness and recovery language into mental health programs. But it has long been clear that not everyone agrees about what is meant by recovery.

Trying to define recovery has always been a challenge.  Not the dictionary definition, which is relatively straightforward but not simple. Some common dictionary definitions of recovery include 1 A return to health 2 Return to a normal state 3 Gaining back something that was lost. The hard part was to explain what recovery meant when related to the issues of mental health and substance abuse.

Many people describe themselves as recovering. By that they mean they are struggling with a chronic progressive disease like alcoholism or drug addiction. They do not believe you are ever cured of a chronic disease condition and continue to remind themselves that at any moment they could experience a recurrence if they were to relax their vigilance.

Some people prefer the term “in recovery.” They believe that recovery is not a passive thing that happens to them but something that requires their active participation. They need to climb on board the recovery train and move towards their goal.

Some old-time AA members describe themselves as “recovered” and they point to the title page to the book “Alcoholics Anonymous” which states that the book is “the story of how many thousands of men and women have recovered from Alcoholism.” They further point out the book say that “we are not cured” but “have recovered from a hopeless state of mind and body.”  The thought that anyone could ever be fully recovered scares other people. They point out that accepting being recovered might lead someone to become careless and think that there is not more work to do on themselves.

SAMHS has suggested the following as a working definition of recovery:

A process of change through which individuals improve their health and wellness, live a self-directed life, and strive to reach their full potential.

SAMHSA delineates four major dimensions that support a life in recovery, Health, Home, Purpose, and Community.

A resiliency researcher tried to explain the difference between recovery and resiliency as – Recovery is coming back from negative consequences and resiliency as the ability to continue to function despite traumas.

A short way to think of this might be – Recovery is the process of getting back to where you were, regaining your life and Resiliency is handling life on life’s terms.

Teens, drugs and resiliency

Some new developments in the area of teens, drugs, and resiliency

Energy drinks:

These drinks have been involved in a large increase in hospital emergency room visits. Many young people and some adults think that energy drinks since they are sold in food and convenience stores are safe. Maybe they are in and of themselves but the way many people are using them is not safe. In a five-year period, hospital emergency room visits involving energy drinks have increased more than 1,000 % from just over 1100 to 13,000. The majority of these incidents, more than half, involved using energy drinks with drugs and alcohol. Males were more likely to mix energy drinks with illegal drugs and alcohol while females mixed them with prescription drugs.

A single can of the stronger energy drinks contains up to ten times as much caffeine and other stimulants than a caffeinated soda. People who consume energy drinks with alcohol are more likely to be involved in accidents as they do not feel impaired and over-estimate their abilities to drive or engage in other dangerous activities. These products often sold as “bath salts” or incense but commonly abuse by younger consumers by smoking, continues to be a problem. Despite analog and similar laws that say that even if you paint the duck red it is still a duck, manufacturers are finding ways to make and market new synthetic drugs that are different enough to escape the net of banned substances.

Join Together, (join together at the partnership at drugfree.org.)  a great source of information on the latest drug trends, reports that 43 states have laws regulating synthetic drugs. Some laws list specific chemicals as illegal. The manufacturers of synthetic drugs keep changing to new chemicals, not on the list. Other localities have listed categories of chemicals as illegal. Under these laws, prosecution is more difficult as the chemical may have legitimate uses that are not related to drug abuse.

Occasionally one of these new synthetic drugs results in a rash of hospitalizations or even deaths. Often these incidents are confined to a small area or a particular supplier of the product. The risks here are real but the problem keeps changing.

Volunteering reduces teen drug use and increases resilience.

In a previous blog, I wrote about the role of sleep in reducing teen problem behaviors and increasing resilience. There is more.

Another simple low-cost prevention measure for reducing teen drug use and improving resilience has been found. Surprising how simple some of these ideas are. The report, again by Join Together staff, reported that kids who regularly volunteer to help others are less likely to use drugs and get into trouble. This sounds a little twelve stepish.

The study they reported about was conducted with rural teens. Rates of drug use among rural teens are rising rapidly. No folks you can’t protect your kids from drugs by moving to the country, and the country folk isn’t just drinking whiskey anymore.

The economy and government budget cuts have reduced or eliminated many after-school programs and activities for kids. There are almost always opportunities to volunteer to help others. Not only did volunteering to help others reduce rates of teen drug use, the results continued into their young adult years, possibly beyond.

Food is also a treatment for teen drug use and promotes resilience.

While I am on the subject, I recall a study that reported that teens that sat down around one of those old fashion dining room table things, no T. V. mind you, that is kids who regularly eat dinner with their parents, they have fewer drug problems, better grades, more resilience and so on.

The conclusion

The risks and dangers for kids continue to grow. We can’t always protect them, probably parents have always been less able to protect kids than the adults would have liked. What we are seeing are research studies that show what a lot of folks always knew. Plenty of sleep, regular family meals, being taught right from wrong, and volunteering to help others, all these things result in a teen who is less likely to take excessive risks and more likely to develop resilience and bounce back from adversity.

Even when I don’t agree with someone I appreciate their interest in my blog. Links are even better. I think it was La Guardia that said, it does not matter what you say about me, just spell the name right, or in this case get the link right. But still – something that happened today shows how easy it is to be misleading when you quote someone else. In this case, they were sort of quoting me.

My blog about our tendency to take the easy way out and give kids lots of meds instead of the personal connections they need to develop was quoted on a website about Medicare. That was nice, only the picture they ran with my blog post did not come from my blog which might make it misleading.

Just so everything is clear, I am very much in favor of parity laws. Paying as much to treat mental illness and drug addiction as we do for physical illnesses is an idea whose time has come. I am also in favor of anything that makes sure health care coverage is available to all.

People who do not have health care end up at free county hospitals and most of the time they don’t get to the emergency room until they are critical. People with untreated mental illnesses are clogging up our jails and prisons and medical hospitals. We pay for all this treatment whether the people have insurance or not. So some form of national healthcare preregistration just makes the system more efficient and manageable.

It seems clear that the country cannot retreat to the days of family farms and paying the doctor with chickens. Our future also does not involve making more things for cheaper prices. Our future as a country does depend on developing minds. We need more Ph.D.’s not more press wood furniture.

Some people seem to think that teachers, especially college ones are a waste of resources. They don’t want to pay for medical services for anyone but themselves. If this country is to stay a world leader we need more teachers, more doctors, and more professionals who work in mental health and substance abuse. We also need more engineers and scientists. Developing these specialties requires easily accessible education as well as physical and mental health care.

All of these things, like the military and the coining of money, are things that need to be undertaken by the government for the good of all. The majority of promising candidates for the service industry do not come from the rich. The best and brightest minds come from among the ninety-nine percent and they need the government at all levels to promote the growth of the service industries and education.

The future is in the areas of ideas and services. The government needs to take the lead in growing these sectors. I am not opposed to businesses having a large role in this effort. Non-profit companies are especially good at efficiently providing these services. But the mentally ill, the drug addicts, and the homeless don’t have money to pay for their care and we all pay for it one way or another.

So the association of a picture of President Obama superimposed with a communist flag was not my sentiment. If anything, I think this administration has been too conservative in accommodating the role of large businesses in the economy.

We made progress in the space race because the administration at that time led the country into a new area. Hopefully, our leaders will lead again into the new service-oriented economies of the next few centuries.

The one bright spot in all this, to my way of thinking, is that at least a couple of Republican candidates for our highest office are now sounding like they are more liberal than the current administration. Maybe this veer in the liberal direction will get our nation back on track.

The hummingbird picture came from Wikimedia. It has nothing to do with this post but then the picture someone attached to my last post quoted on their website had nothing to do with my post. So now things are about even.

Is your child taking too many meds? Are you?

Ever wonder about the amount of medication, especially heavy psychiatric medication, which is being prescribed to very young children, especially foster children? I know I do. How did we get so many children who need that level of medication? What should you think if your child or the child you care for was prescribed that kind of medication?

There was an article on the internet this morning about the large number of prescriptions for antipsychotics to foster children.  These kids were being given not one medication but multiple medications and many of those medications have serious long-term side effects. Some of these side effects do not go away even after the medication is discontinued. So is all that medication really necessary?

The usual warning applies here. No matter what you and I think, it is unwise to stop, start or alter the dose of a medication without talking with your doctor first. If you are not confident with that doctor’s opinion get a second opinion. It is not safe to play doctor and change the meds yourself. But you should question things if the medications are not working or if it appears to be creating serious side effects.

So why so much medication in children? And why foster children in particular?

Now we have been hearing tales about facilities that overmedicate residents to keep them manageable for a long time. Mary Jane Ward in her book “Snakepit” describes patients in a mental hospital who were heavily medicated primarily to keep them from causing the staff problems. We also hear stories about jails and prisons using lots of meds to keep inmates from causing trouble. But is that sort of thing happening to kids? And why foster kids?

Maybe the reason is that foster kids are the ones that have the most problems? Are children in foster care more likely to have serious mental illnesses? There are two reasons to think this is not true.

The study reported that when kids in foster care were compared to other kids who were receiving Medicare the foster kids got a lot more medication that the kids who were not in foster care. Secondly, the kids in foster care should be the ones who would respond to a secure home situation. Foster care is usually looked at as a housing problem, not a mental health problem. So the seriously mentally ill children should and often do end up in group homes and institutions where they can be managed with less, not more medication. So what is going on here?

One problem is that as a society we are increasingly relying on medications, drugs if you will, to fix all our problems. Despite all our programs to get people off drugs, there are countless commercials that try to convince us to go to the local drug store and get some pills to cure all that ails us. Try counting the number of pro-drug ads on the television on any given night and then tell me that we as a society are opposed to drugs. In this country we love drugs so much, are so convinced there is a chemical that can cure whatever is wrong with us that we now want all our children to take all the drugs they can. We just want them to get their drugs from a drug store not a drug dealer in an alley.

So why are children being given so many drugs? To relieve suffering? Not a chance!

Kids are getting prescribed drugs to make them smarter and to make them behave.

Kids are being prescribed powerful stimulants for ADHD. Even kids with mental retardation and developmental delays are getting ADHD medication. This makes me question the accuracy of some ADHD diagnoses. And if the pills make one kid smarter shouldn’t all children take them?  Here is hoping you read that blog post also.

The other reason kids get so much psychiatric medication is to make them behave. I thought not always behaving properly was a symptom of childhood? Can’t we teach them to behave without sedating medications?

Children are being given lots of sedating antipsychotics mostly because their behavior is causing some adult a problem. Some of my colleagues will argue that if the child does poorly in school or gets in trouble for bad behavior they will end up suffering so by giving them medication we are preventing suffering. For me, that would make sense if there were no other alternatives. But there are other options.

Many of the problems of children for which we give them medication can be controlled, even cured by giving them attention in the proper way. Therapy and counseling are especially helpful for treating many emotional problems. ADHD can be treated by training parents as I wrote about in a previous blog. But far more kids get a prescription for a medication than are getting a session with a caring adult.

Yes, there are professional counselors and therapists; I do that for a living. But the benefits of other adults in a child’s life should not be underestimated. Teachers, grandparents, natural or foster, and friends all are helpful in a child’s learning emotional regulation. So why do we reach for meds first instead of human contact for all the emotional and behavioral problems?

Some people have suggested that counseling is just too expensive. I am not buying that argument and you won’t either if you see how much agencies are spending on medications.

Despite all the things we have learned over the last couple of centuries about the benefits of counseling in overcoming life’s problems we as a society still seem to think that if something bothers you there should be a drug to fix that.

My conclusion from all this is that a lot of kids and some adults are being given lots of medication instead of human contact. It appears that we as a society and some people individually just don’t care enough to bother with anything more than a pill to make the kid behave.

6 Rules for surviving your teen

When the teen years arrive, armed combat begins.

How do you survive your teen unharmed?

Sometime between eleven and thirteen that cute cuddly kid turns into an unrecognizable scratching clawing creature. Once in a while, the parent gets away unharmed but not often.  Parents say they don’t know what happened to their child. They don’t recognize this kid. So what happened?

How do you survive the teen years and is there a life after teens? Here are some ideas; maybe we could call then rules for those years.

1. Don’t try to hold the lid on the boiling pot.

You have spent a decade on more raising a generally civilized child, as the teen years approach and the emotions boil parents often make the mistake of clamping down. Suddenly the cute little daughter who everyone likes – well – the everyones who likes her, are no one you would want around. The result is trying to keep her in. “No dating till you’re thirty,” the parent says. Then the battle ensues. Parents try bolting the windows in her room shut, but like a magician, she somehow gets out. Parents put kids on monitoring. Call me every hour. The kids retaliate with more excuses than a congressman.

Controlling a teen, especially an older one by force is likely to wear you out and have no effect. Sometimes parent’s efforts to make the kid behave turn into violence. Sometimes the parent resorts to hitting the child, always a bad idea. You may discover you no longer have it in you to go ten rounds with a younger and stronger opponent. I have seen parents seriously hurt by their kids. The other, more serious problem with using force on a child is it teaches them to use force and there is no end to how far this will go. So rather than trying to keep the steam in the kettle by holding the lid down, try directing the steam where you want it to go.

As the teen approves adulthood parents may need to learn to discuss and even negotiate things with their offspring. I am not saying let the kids take over the house, but you do need to teach them how to handle more adult responsibilities. By sixteen or seventeen you should have taught your child the difference between right and wrong. If you haven’t it is probably too late and someone with more control than you will need to take on the job, someone like the police or parole. You need to keep up hope that the child will survive, unharmed, the episode of moral amnesia that so many of them experience.

2. Do not try to overprotect them!

You spent ten or more year protecting your child, every “good enough parent” does. Suddenly the experts tell you to stop trying to protect them. I know they will be sixty and you will still feel protective towards them but the teen years are the time for loosening the restrictions, not tightening them. You had to let them ride their bike without your hand on the seat, now you need to let them try some more adult things.

Every night in crisis centers around America we see kids whose parents have always been supportive or permissive, who suddenly engage in a life or death struggle for control with their teen. Kids who had no curfew now chaff as the parents set limits.  Parents worry about drugs, alcohol, driving, and mostly sex. They try to keep their kids safe by keeping them away from the risks – that won’t work.

One day they will turn eighteen and then they will be allowed to make all their own decisions. Some kids start before that. There is no magic cloak of maturity you can give them on that occasion.  You need to begin now teaching them how to be responsible adults and one way they learn that is to try things and see what works and what doesn’t. Increasing rules and restrictions may feel like it is protecting your child but it may also be delaying the growth of maturity.

3. Notice when your child does something right.

Many kids report the only time their parents notice them is when they are correcting them. Constantly finding fault with teens is not likely to make them perfect. It often results in kids who are highly anxious, afraid to do anything because they are sure they will never be able to do it right, or you get kids who give up trying. If the only way to get your attention is to mess up, they will mess up on a daily basis. They are after all giving you what you are requesting. You will get more of whatever you attend to.

Now I am not suggesting hollow praise here. Kids can see right through praise that was given to increase self-esteem but which they see as just something everyone is able to do. What I am suggesting is that you need to pay close enough attention to your child to know when they do something noteworthy and then let them know that you noticed and approved of that.

4. Be their parents, not their friends.

Kids should be kids and parents should be parents. Sharing your drugs with your child does not make for a good relationship. It makes for a child who does not know how to observe boundaries. And even worse are the parents who flop back and forth. One day you want to be the kid’s best friend, maybe even keep a secret from the rest of the family, the next they come down on the kid with all their force because the kid is not doing what they want.

5. Know the difference between rewards and punishments and bribes and abuse.

Lots of people in our society don’t seem to know this one. From the way we see celebrities and politicians acting you would think they are the same thing. They are not.

Rewards and punishment should be directly related to the person’s actions. For adults, this is easy to explain. If I show up for work on time and do my job I get a check. If I am late, I get docked some pay. If I keep coming in late I may lose my job. Parents get this confused and send the child to their room for getting bad grades. Bad grades should get more study time. Going to your room should be a punishment for not behaving around others. See there is some connection between the two.

Do I need to say that some of the punishments I see require me to report the parent to child protective services? Don’t ever let the punishment get out of proportion to the action. When it does it can turn into abuse. This is especially true of physical punishment and name-calling. Calling your child stupid will not improve their grades; it will make many of them stop trying.

6 Pick your battles

Parents, especially of late teens begin to get desperate. Time is running out to teach your child how to behave, especially if you have a strong feeling they should behave exactly the way you want them to. So every day becomes a battleground. The chances that your child will turn out perfectly are not especially good. They all have their flaws. So do their parents.  Unless you really like to fight, day and night, I suggest you reserve your line in the sand efforts for the really big things. Which is more troublesome, your teen’s messy room, or their drug habit?

7 “New Drugs” parents should be aware of

7 “New Drugs” parents should be aware of.

New drugs and new patterns of drug use continue to emerge. When I first started talking about this in my class for drug and alcohol counselors the thought entered my mind that giving out information on new drugs might encourage their use. I didn’t need to worry about that, the people who want to use them knew about them far before I did. But now that there have been a few overdoses I believe it is important for parents and professions to be aware of these new trends in drug use. So here are six new drugs and one new drug use trend that are beginning to rival the old drug use problems. For up to the minute information on these drug use trends you only have to search the internet.

1. Khat

Khat is a stimulant plant from the Middle East, another of the results of our involvement in wars there. The leaves are chewed while still fresh and moist and are a mild stimulant similar to Coca leaves. The plant and the fresh leaves are rare in the U. S. The synthetic version is becoming more common.

2. Methcathinone

Methcathinone is a synthetic and potent laboratory-produced version of the Khat plant’s active ingredient. It is not illegal or regulated everywhere yet and is growing in popularity. Results are reported as being similar to Methamphetamine.

3. Bath salts.

These are not the kind of bath salts your grandmother might have used. These drugs are being sold in “head” or “smoke” shops not bath boutiques and the label is certainly a misnomer. Common names include such benign-sounding names as “Ivory Wave,” “Purple Wave,” “Vanilla Sky” and “Bliss.”

They are used by smoking and can contain a variety of chemicals. Join Together reports that DEA has placed a temporary ban on three ingredients used in the manufacture of bath salts, Mephedrone, MDPV, and Methylone.

As fast as one ingredient is made illegal the manufacturers switch ingredients. Overdoses can be particularly nasty and use may result in psychoses or death. To re-quote “What a price to get your kicks.”

4. Synthetic Cannabinoids.

These are best known locally under the brand names of “Spice” and “K-2.”  This can be most any dried vegetable material, commonly parsley which has been coated with a synthetic Cannabinoid. There are 300 different chemicals involved so far and more are sure to be discovered. A few have been made controlled substances, mostly this means they are illegal in the U.S. As fast as one is banned another variety comes into use. These are not benign chemicals. Overdoses and toxic results have been reported including hallucinations that have not gone away after withdrawal from the drug.

5. Salvia Divinorum.

An unusual member of the sage family originally from Central America it appears to be the only member of the sage family with psychoactive properties. It was used by Native Americans in religious ceremonies and does not appear to be especially dangerous when used that way. When combined with other drugs, especially synthetics and alcohol the results are reported as being unpredictable. Since stimulants and depressants are the most popular drugs, consciousness-altering drugs like sage have not caught on in popularity the way Methcathinone and bath salts have. As with most dried herbal products the potency and ingredients can vary considerably.

6. Kratom.

Bet you thought I made that up? This has nothing to do with Superman. It is a tree, originally native to South East Asia. The leaf is reported to have both stimulant and depressive properties. In some places it is illegal and in other places, it is totally unregulated. At high doses, it has been reported to have effects similar to morphine. Some of the trees are now in the U. S. but most of the use is by buying leaves and preparations from the internet. The tree does not grow well in cold climates so most of the cases reported are from Florida. Like all other drugs, it is likely to spread over time.

7. Smoking of Heroin by teens.

This is a new twist to an old drug. This trend is occurring in the wealthier and more affluent parts of town. Abuse of pills is now epidemic. Teens have ready access to powerful painkillers. Sometimes these have been prescribed to them for injuries but often these pills are being taken from parents and grandparents medicine cabinets. After a short time, opiate addiction develops. Unable to get more pain pills an exceptionally large number of teens have taken to purchasing heroin to replace the pills. At first, they may be induced to smoke the heroin, thinking that this differentiates them from the drug addicts who use needles. The high price of the drug and the larger quantities needed when smoking result in most switching to needle use. Heroin is consuming a whole new generation.

By the time I get this posted it is likely there will be additions to the list. I hope this helps in the way of information. The only antidote I know of for an increase in drug addiction among our children is parental and societal involvement with kids. Happy, healthy kids are less likely to become addicted and they are more likely to turn to adults for help. Kids with mental health problems, who are estranged from their parents, are at increased risk. Trying to keep drugs out of our communities does not seem to be working as you can see here new drugs of abuse will keep entering our society. The only hope for taming the dragon of addiction is early intervention and treatment.
