Should you worry if you get pregnant by a Methcathinone user?

Anything that damages an egg or the sperm could result in birth defects, age, the physical health of either party, and so on. There is always a risk of spontaneous mutations and genetic damage. I have not seen much good research that indicates that one drug is better or worse than another when used by the father.

ANY drug use by the mother during pregnancy is risky. Sorry ladies. Doctors even take pregnant women off some prescribed medications. Alcohol is the biggest risk factor for preventable birth defects we know of. The only fully safe amount of alcohol a woman should consume during pregnancy is NONE!

If you are worried about the drug use of the father of your child you have more to worry about than possible birth defects. YOUR BABY’S DADDY MIGHT BE A DRUG ADDICT.

Pick your baby’s daddies and mamma’s wisely, you can break up with your partner but your child’s other parent is forever.

Stress and time off from work – Morning Question #18

Short answer – Yes, no, and maybe.

Yes. – Stress is diagnosable – IF the person’s reaction to the stress is extreme and they are unable to work, have relationship problems, or are upset about the way the stress is affecting them. It could be Acute Stress Disorder, Posttraumatic Stress Disorder, or possible an Adjustment Disorder.

No – not everyone who has stress at work should get a diagnosis. Everyone has some stress, even good things like a new job or a promotion can be stressful. So just having stress is not enough for the diagnosis.

Maybe – Maybe you can take stress time off with pay, maybe you won’t get paid for time off.

Time off from work is a legal issue. If you have leave coming or qualify for disability then you might get paid time off. Sometimes a counselor will suggest it would be good for you to take some time off even if you do not qualify for paid time off. Self-employed people probably will not get any paid time off but the counselor may recommend it anyway.

Check the laws in your jurisdiction and the rules for your workplace regarding the issue of the time off being paid.

Do you have to tell what your therapist license status is?

Morning Question #17

YES! Sorry folks I know sometimes this is a pain.

If you are an intern say you are an intern. If you are a trainee say so. Once you say what you are, you may end up needing to spend some time explaining the designation. When people call me Dr. Miller I have to explain that I am a Therapist and Counselor and that I have a Master’s Degree, not a Ph.D.

People with a doctorate may be able to call themselves “Doctor” but they need to explain that they are Ph.D.’s or PsyD’s not medical doctors. In most places, Ph.D.’s can’t prescribe and we all need to help clients understand what we do and not mislead the public.

If this bothers you, go into something with a less rigorous code of ethics.

Can one person be a support system?

How many people make a support system?

Morning Question #16

Having a close, significant person as a support system can be extremely helpful. People with a serious and persistent mental illness who are in a long-term supportive relationship are less likely to end up back in the hospital. There are several reasons why a one-person support system is risky for them and for you.

Expecting one person to carry the full load of supporting you is an awful lot to ask. It is too much for someone to care for your needs and to be in a close relationship with you. How does that person get their needs met? If you need a support system, can you be fully present to meet your partner’s needs? Having people other than your partner in a support network increases the support you can call on and avoids pushing that one person who is around you all the time to the breaking point.

More people in your support system spreads the burden around and increases the joy of being able to help each other. Building a support system is important, so is making sure they are supportive.

Support people are often relatives or close intimate partners. Having someone to love and who loves you can be very supportive. No relationship is ever conflict-free. If you and your partner have a disagreement, if there is a fight, you risk your support system being unavailable just at the time you most need one.

We tend to be attracted to and close to people like ourselves. There is no reason why two people who have depression or any other mental illness can’t be in a relationship. If your partner has issues also they may not always be available or able to cope with your issues.

Too many people in your support system may be just as much of a problem, as too few. It is difficult to stay in contact with many people. A support person should be someone you know well and who knows you well.

Is Dysthymia better in the morning or worse?

Which depression is worse in the morning? – Morning Question #14

Having more depressive symptoms in the morning is a characteristic of “Atypical features” of a mood disorder. Atypical does not mean unusual it means “not melancholy.”

Atypical features include the hibernating-like-a-bear symptoms, overeating, sleeping too much, and being tired and grouchy in the morning. With atypical features, the person may be able to feel better temporarily if something they really like happens, but the happiness does not last long. They may also feel better in the evening but by morning the depression comes back.

Atypical features can occur during episodes of Dysthymia, Bipolar one or two Disorders, and Major Depressive Disorder. In practice, the only time I remember seeing this on a file is as part of the diagnosis of a Major Depressive Disorder but having atypical features increases the risk that this will eventually turn into a Bipolar disorder.

If your mood is customarily worse at a particular time of day, make sure you are eating and sleeping well, that there are not environmental problems like relationship issues that are causing this, and then seriously consider consulting with a medical doctor or psychiatrist. If the doctor rules out any medical problems then some counseling should help.

Is anxiety a mental illness?

Anxiety is a disease? – Morning Question #13.

Anxiety can be either a normal emotion or a mental illness. A little bit of anxiety is protective. It alerts you to danger. If they are shooting at you, get nervous and take cover. Bravery is not a lack of anxiety it is taking action in spite of your fears.

Anxiety as a mental illness is the result of having your “anxiety volume” turned up too much. If fear of everyday things or excess fears about things that have a low probability of happening is interfering with your life then you might have one of the diagnosable anxiety disorders.

Anxiety becomes excessive if it keeps you from having a productive life. If your anxiety is preventing you from holding a job, doing positive activities, having friends or if it is making you miserable you may have a treatable condition called an anxiety disorder. That is the time to see a professional and find out about your condition and to get help.

Do therapists have to report a crime?

Do counselors report crimes? Morning Question #10

The general answer is NO! The more you can talk to a therapist about the more likely you will be helped to change your behavior. Therapists have a legal and ethical duty to NOT repeat what you say. Any exceptions to that rule are determined by law. See my posts on “How Much Should You Tell a Therapist?” or “Why pay a therapist when you can just talk to a friend?”

So could a government somewhere make a law that the therapist had to report a crime? Sure could.

There are laws in many places that protect patient and psychotherapists communication, very similar to doctor and patient privilege. The relationship between a patient and a psychotherapist is held to be special, like that between a person and a religious priest.

The principle exceptions to not having to report crimes are, abuse of children, the elderly, and the disabled, (see – Does abuse of seniors and the elderly get reported?), and if you are suicidal or plan to kill someone else. Also if you are being investigated by homeland security we may have to report. As long as homeland security confines their investigations to known terrorists I am OK with this, as it falls under that duty to protect other intended victims. Some therapists have worried that this could be interpreted as needing information of a particular religion or political party and we would mostly be opposed to that sort of reporting.

Short answer: past crimes usually do not get reported, future and ongoing crimes like abuse or a plan to kill probably will be reported.

Morning Question # 8 – Which personality Disorders can’t read other people?

Which Personality Disorders can’t read other people?

None really. The closest would be Schizoid Personality Disorder, they can read people they just don’t care for people, any people. They are the classic hermits and loners.

I think you are asking about the Autism Spectrum. Autism, Asperger’s, and Pervasive Developmental Disorder NOS. Next year with the DSM-5 these will all become part of a new label Autism Spectrum Disorders. People with these disorders or varying degrees of this disorder have difficulty reading other people.

When shown pictures of people’s faces they can’t tell the angry person from the calm person. They also have difficulty reading lots of non-verbal clues. So they don’t understand when someone likes them and wants to talk or is bored and wishes they would go away. This creates all sorts of social problems.

One new treatment approach consists of teaching them all about nonverbal clues to behavior. More on this treatment coming up in a future post.

Morning Question #7 – Toxicology not picking up bath salts?

Do drug tests find Bath Salts?

The jury is still out on that one. Bath salts can be made from hundreds, maybe thousands of chemicals. Standard drug testing and hospital toxicology screens just are not designed to find a whole lot of drugs that are rare or unusual. Drug users are constantly looking for ways to beat drug tests and the labs look for ways to catch the cheaters. The wheel keeps going around. I think that there are lots of kids and older drug users, playing with synthetic drugs these days, and those drugs do not show up on standard drug tests.

If the person asking this is referring to those high-profile cases that make the news. Those CSI types are pretty good at finding things so if they say something was not there, I am a lot more confident it wasn’t there than when someone is reading a standard 5-panel instant drug test.

Morning Question #6 – Could a father’s meth use cause Schizoaffective Disorder?

Schizoaffective Disorder and Meth use.

Schizoaffective Disorder is an unusual condition, though I think it is underdiagnosed. Think about traveling through a swamp full of lots of muck but here and there are some small islands of land. Sometimes you have one foot on land, sometimes two, and other times none. People with Schizoaffective Disorder can have episodes of psychosis, episodes of depression, or Bipolar disorder, sometimes both and sometimes neither. Depending on who sees them and when they are seen the diagnosis can change all over the place.

Mental Health conditions do not seem to be caused by any one gene. Most likely they are caused by the interaction of many genes, risk factors, environmental influences, and so on. Dad doing drugs is not likely to have caused that. Sometimes dad was doing drugs because he had a mental illness or there was a history of some disorder in the family that put him at risk.

Mom’s drug use during pregnancy is another matter. The more she uses the bigger the problem. The day she uses can determine what part of the baby was developing that day and what was affected. Mom’s drug use can result in mental retardation or learning disabilities but so far I have not seen any research out there that says the parent’s drug use before the child was born changed the child’s genes and caused a mental illness. If someone has seen that please leave a comment.
