How drugs are classified for legal purposes.

The Controlled Substances Act of 1970 created another way to classify drugs. In another post, I talked about some of the ways drugs can be classified, by their origin, by their medical effect, or by the experience the user has. For legal purposes, some drugs are scheduled on one of five separate schedules of controlled substances.

The criteria for placing drugs on a particular schedule include whether they have an accepted medical use, their potential to be abused, and the likelihood they will produce dependence. Many people refer to drugs with a high abuse potential as narcotics. Narcotics technically are drugs that will put you to sleep, knock you out, and may potentially cause death. Many of the scheduled drugs are stimulants or Hallucinogens. When talking about the legal regulation of drugs, it is important to know that the correct term is, controlled substances.

Here’s a brief description of the five drug schedules and some examples of drugs on each schedule.

Schedule I drugs.

These drugs have an extremely high abuse potential. They are likely to cause dependence, have withdrawal effects, and produce cravings. They are not currently accepted for medical use in the United States. These drugs are not believed to be safe even when used under medical supervision.

An example of the need for scheduling some drugs on schedule one is illustrated by the drug thalidomide. This drug was approved and widely used in Europe, where it was often prescribed to women for the treatment of morning sickness. There was a good deal of pressure for it to be approved in the United States. One doctor at the FDA resisted the pressure to approve the drug for use in the US because of safety concerns.

After this drug was used in Europe for some time, it was discovered that the drug would produce serious birth defects when women who were pregnant were exposed to the drug. Even handling the medication could produce birth defects. Today this drug is used for treating cancers, but it continues to carry warnings that women who may become pregnant should not come in contact with this drug.

Some examples of drugs currently on schedule I include, heroin, peyote, psilocybin, ecstasy, LSD, methcathinone, marijuana, and most other products containing THC.

Schedules II drugs.

These drugs have a high abuse potential. They must have a recognized medical use, though it may require severe restrictions. Abuse of these drugs can result in either physical or psychological dependency. To prescribe these drugs, doctors must have a DEA number and use a triplicate prescription pad. One copy of each prescription stays with the doctor; one is given to the patient who must take it to the pharmacy, the third copy is sent to the DEA.

Examples of schedule two drugs include morphine, codeine, methadone, other opiates, Ritalin, methamphetamine, cocaine, PCP, and many other similar drugs.

Schedule III drugs.

Drugs on this schedule are more widely used for medical purposes. There are less likely to be abused than drugs on schedule II. These drugs can still be abused and may produce a mild to moderate substance use disorder.

Examples of schedule III drugs include Vicodin, Tylenol with codeine, Marinol a synthetic capsule containing THC, Ketamine, anabolic steroids, and similar drugs.

Schedule IV drugs.

These drugs have wide medical applications in the US. They have a low potential for abuse when compared to drugs on schedules I, II, or III. People may build up some tolerance to these drugs and experience withdrawal, resulting in physical or psychological dependence, but these drugs are less likely to produce cravings than the drugs on the earlier schedules.

Examples of schedule IV drugs are Darvon, Valium, Librium, Ativan, Xanax, and similar drugs.

Schedule V drugs.

These drugs have many accepted medical uses in the US. They have the lowest abuse potential of all the scheduled drugs. Abuse of the drug may lead to some types of dependency. Some drugs on schedule V are available in both prescription strength and an over-the-counter version. For example, some pain relievers are available over-the-counter in two hundred milligrams strengths, the stronger six hundred and eight hundred milligrams versions require a prescription.

Just because drugs are on schedule V does not mean they are totally safe. Many people abuse over-the-counter pain relievers. Taking too many of these medications can result in permanent damage to either the liver or the kidney.

Some problems with drug schedules.

Despite the movement towards legalizing marijuana for medical and recreational use, the federal government continues to keep it on schedule I. This creates conflict between the federal government and state or local governments. Doctors who prescribe medical marijuana are likely to lose their DEA numbers. Doctors who work in hospitals or chronic pain clinics are unable to prescribe marijuana without losing their ability to prescribe stronger opiate pain medications.

Heroin continues to be a schedule I drug. This has prevented doctors from prescribing heroin to people who are addicted to opiates. In other countries, addicts, can receive their drugs by prescription and can administer them in safe injection sites where the used needles are properly disposed of. Because heroin stays on schedule I, addicts are prescribed methadone a drug which is even harder to withdraw from than heroin.

Warning signs you are headed for burnout.

Burnout can derail your life.

Burnout is usually associated with the inability to continue to work. Burnout can result in

poor work performance, getting fired, or employees quitting. Burnout has also been reported in caregivers for the seriously ill. If your emotional exhaustion is interfering with your ability to work, and that work may include caring for ill family members, don’t wait for the breakdown. If you have any of the symptoms alone, seek help.

Whenever you experience physical symptoms, it’s wise to start by seeing a medical Doctor and rule out a medical cause for your symptoms. But if the doctor can find nothing organically wrong that doesn’t mean you should ignore the problem. When you’re under stress over a long period, your emotional system takes a beating. Here are some warning signs you may be headed for burnout and no longer be able to cope with the life you’re living.

Extreme low energy for no apparent reason may be burnout.

Burnout often begins with emotional exhaustion. When things come out you faster than you can cope with your emotional system can become stressed out. You are suffering from emotional exhaustion pay attention and make some changes in your life.

Reduced ability to sleep is common in burnout.

Whenever you notice a change in your sleep patterns, look for a cause. Having too much on your plate, getting to bed too late, and getting up too early puts you at risk for emotional exhaustion and work burnout. But if you go to bed early and still can’t fall asleep, if you have frequent awakenings or wake up far too early in the morning and can’t get back to sleep, this suggests that something is impairing your sleep. Don’t ignore changes in sleep, do something about them.

Loss of appetite can be a sign of depression.

If you find that you have trouble eating, losing weight for no apparent reason, think about the emotional stresses taking place in your life. Loss of appetite can be a symptom of a serious major depressive episode. Your body can’t run well without healthy fuel. Don’t wait till your body has paid the price before you do something about reducing your stress.

Loss of ability to focus, concentrate, or remember may be from burnout.

People who are suffering from burnout find they will have difficulty with paying attention and focusing on tasks at hand. As burnout becomes more severe, there are less and less mental resources available to stay on task. Burnout by itself will impair your thinking. Add in the loss of sleep and changes in appetite and your ability to function can decline dramatically.

Burnout causes increased forgetfulness.

People suffering from burnout tend to forget. It’s the mind’s way of protecting you from too much to do. Pay attention to this warning sign.

Being both physically and emotionally drained are symptoms of burnout.

Burnout begins with emotional exhaustion but frequently progresses to leave you physically drained also. If getting extra sleep on the weekend doesn’t bring you back to normal pay attention to that worn-out feeling. Don’t wait for the nervous breakdown before acting. People who reach the more severe levels of burnout may be unable to return to their job or may even be unable to work in the future.

Burnout causes Loss of motivation.

People can get through some very difficult times if they have a meaning or purpose in life. If you stopped caring about what you do and why you do it life will become much more difficult. Think about ways you have motivated yourself in the past. Study motivation techniques. But if you find you are just to worn out to care you’ve reached a level of serious burnout.

Frequent illnesses can be the result of burnout.

Unhealthy stress increases the chances you will become sick. Stress reduces your resistance to infection. Burned-out workers use more sick days each year than there are emotionally healthy co-workers. Don’t lose a large chunk of your life to burnout related illnesses.

Physical symptoms with no apparent medical cause may be burnout.

Burnout related stresses put a heavy burden on many of the systems of the body. Burnout along with anxiety and major depression overlap a great many physical issues. Check with the doctor first but also consider ways to reduce the stress and to learn new more effective ways to cope with ongoing stressors.

Burnout could cause constant, high anxiety.

If the anxiety is your constant companion but others around you don’t perceive any real danger you may have become hyper-vigilant always on the lookout for something that could go wrong. Excessive levels of anxiety are another symptom of burnout.

Burnout causes an increase in anger and irritability.

As people’s level of burnout increases, they become harder and harder to be around. If you find it difficult to get along with people at work who used to be your friends, ask yourself if you’re becoming more irritable. Don’t wait for your irritability to become anger and your anger to result in doing things you regret. Coworkers suffering from burnout are a major source of workplace violence. Seek help before you do something you’ll regret.

Does your self-esteem need a boost?

Many people describe themselves as having low self-esteem, and yet the things they are doing and the way they are doing those things are reducing their self-esteem rather than improving it.

If you would like to grow your self-esteem here are some behaviors to improve your self-esteem, and some things to stop doing that may be damage your self-esteem.

Doing more worthwhile things builds self-esteem.

One cause of low self-esteem is inaction. Doing nothing is hard on your self-esteem. People who are active, living life, build up their self-esteem. Focus on doing things you can be proud of. Rather than aiming for huge world-changing actions, try to make each thing you do throughout the day something you feel good about.

Focus on the positive not the negative.

Only paying attention to your errors is a sure self-esteem deflator. If you only count the negative you build up a wall that prevents you from seeing your accomplishments. Pay particular attention to the positive things that happen each day of your life. When something good occurs, pause and take special note before that accomplishment disappears from sight.

Develop a positive support system.

Surround yourself with people who feel good about themselves and about you. Having negative people in your life is sure to lower your self-esteem. Positive people build you up; negative influences pull you down. Maximize your helpful support system.

Increase your self-esteem by learning to love yourself.

Learn to love yourself exactly the way you are. You are a worthwhile person because of who you are not because of the things you do. Learn to accept yourself, like yourself, and enjoy spending time with you. Everyone needs a best friend. Become your own best friend, and other friendships will follow.

Treat yourself the way you want others to treat you.

Be kind and gentle with yourself. Beating yourself up will not make you a better person. The way you treat yourself becomes the model for the way others will treat you. Include time for a healthy lifestyle in your schedule. Eat healthy food, drink plenty of water, and when you’re tired allow yourself to rest and recharge. Don’t engage in self-harming behaviors.

For more self-esteem stop the insults.

Don’t call yourself names. Calling yourself stupid or fat or any other insult will destroy your self-esteem. Learn to view your shortcomings as improvement opportunities. Rather than call yourself stupid, tell yourself that you, like all other humans, sometimes make mistakes. If there are things you don’t know, learn more about them, get more education. If you’re unhappy with your physical condition, see a medical doctor, work with a counselor, and begin the program of self-improvement.

Remember the compliments.

Never getting a compliment undermines self-esteem. Complements are gifts, learn to give them, and to accept them graciously. Give honest compliments. See the good in yourself and others. Don’t lie to yourself or pretend you accomplish things you never did. Do learn to recognize the progress you make in your life.
